RECOMMENDED ACTION (Motion):
Authorize the City Manager to execute the $22,729.43 Change Order No. 4 and the $22,337.69 Change Order No. 5 for the Meyers Road Extension Project (CI 17-001).

EXECUTIVE SUMMARY:
Change Order's No. 4 & 5 for Modifications to the Meyers Road Extension Project. All contract changes including Change Order's No. 4 & 5 represent work additions totaling $131,655.20 and an overall contract increase of 1.8 percent.

BACKGROUND:
As work progresses on the project, the following modifications have been identified and negotiated with the contractor:

Change Order #4, for a total of $22,729.43 includes the following:
· $18,147.14 for changes to the PGE power service connection for the new illumination on Meyers Road. During recent construction meetings (after bid opening) PGE provided more direction on their requirements for the street lighting needed to be completed which required plan revisions and additional work (conduits, additional transformer foundation and associated interconnections)
· $4,582.29 for additional ODOT Bond in 2020. This was an added requirement by ODOT after both the ODOT construction permit issuance and the project bid opening.

Change Order #5, for a total of $22,337.69 includes the following:
· $-5,331.00 credit for removal of unnecessary equipment in the traffic signal cabinet. During recent construction meetings ODOT provided more clarification on their traffic signal equipment needs and authorized the removal of equipment specified in the plans.
· $17,086.04 for changes to the PGE power service connection for the traffic signal at Hwy 213 and Meyers Road. During recent construction meetings (after bid opening) PGE provided more direction on their requirements for the traffic signal power service connection which required plan revisions and additional work (sidewalk demo and replacement, additional conduit, n...
